The National Transportation Safety Board in the USA issues the findings of its report into the death of superstar blues-rock guitarist Stevie Ray Vaughan in a helicopter crash two years earlier in Alpine Valley, Wisconsin. The report determines the probable causes of the accident as 'Improper planning/decision by the pilot, and his failure to attain adequate altitude before flying over rising terrain at night. Factors related to the accident were: darkness, fog, haze, rising terrain, and the lack of visual cues that were available to the pilot.'
